ACNE SCAR TREATMENT : SUBSCISSION
Skin Experts worldwide agree depressed acne scars are the most commonest variety of scars seen on almost 90% acne patients. Of these the ROLLING and BOX SCARS are the ones that respond best to Subscission.
What is Subscission?
Subcision is a procedure used to treat acne scarring, specifically depressed acne scars.
Depressed acne scars are caused when fibrotic strands pull the skin downwards, creating the superficial pits.
During treatment a small needle cuts these fibrotic strands thereby releasing the attachment of the scar, allowing the skin to rise.
How is the procedure done?
After thoroughly cleansing the skin and marking the individual scar borders, a small needle is inserted just below the scar skin.
The sharp edge of the needle is moved in different directions to cut through the fibers that pull the scar downwards creating depressed areas.
This creates a small bleed in the treated area which is a desirable as this blood clot is what lays the foundation for the newer collagen to be formed.
Bleeding is controlled using pressure and skin is sealed using an antibiotic cream and a sunscreen.
What is the downtime with Subscission?
➡Since there is bleeding, a Bruise seen on the skin is inevitable which will last for an average of 4-7 days.
➡There is redness and Swelling soon after the procedure which also persists for 2-3 days.
➡Pain during the procedure is taken care of by the anaesthetic used but after the effect weans off slight discomfort may be perceived.
➡Overall appearance of the skin demands a break from work at least unto 24-48 hours (which is why it is commonly done on weekends).
What is the after care involved ?
As it is almost like a small surgery the aftercare is mostly designed to prevent Discomfort, Infections and any kind of complications.
✴️ Simple Post care instructions are:
✅Use a mild cleanser, avoid friction.
✅Avoid Make up for 48 hours.
✅Avoid Steam, sauna, swimming, Gymming for 48 hours.
✅Restrict sun exposure wherever possible and use sunscreen regularly.
✅Use you antibiotic cream as advised.
✅You may be given some skin supplements to promote healing.
When do you see the results ?
Results are apparent soon after the treatment as the scar looks lifted.
📌 However it is the swelling and the blood clot that is lifting the scar for now which gradually goes down after 4-7 days.
Over a period of 6-8 weeks this blood clot is replaced by new collagen fibres laid down by the body which then account for the permanent lifting of the scar.
If a larger area of the face is affected your dermatologist will plan the treatment over 2 -3 sessions.
🚨 Are the results Permanent ?
Results in scar reduction after Subscission are PERMANENT.
✅ Once treated the skin looks smoother and firmly lifted making the scar less visible.
✴️ How many sessions are required ?
Ideally 1 session does the job but for various reasons the doctor may choose to divide the treatment into 2-3 sessions.
Also repeating a subscission at times may be necessary to treat scars at different depths.
🧑⚕️ All in all sub scission is an effective treatment to begin with for any kind of scars- acne, chicken pox or post traumatic.
✅ “ I choose this as my step 1 for most of my acne scar patients since this also helps my further treatment plans more effective”- says Aesthetic Expert Dr Sonam.
